Vibration at 2500 RPM's and greater

I have a 89 LX and at 2500 RPM's and greater in 5th gear bad things start to happen. At 2500 it comes and goes and comes and goes. its like every 1-2 seconds almost like a bad tire. I know its not the wheels or tires because it did this before I replaced them with new wheels and tires. Anyways if you increase your speed and RPM's from there on up it becomes constant and gets even worse the whole thing vibrates and its loud and as you are accelerating down it vibrates until you get back down below 2500 RPM's. I had the driveshaft balanced u joints are good just rebuilt the motore and tranny. Now it did all this before I rebuilt the motor and tranny and replaced the wheels and tires. I hope the motor isn't out of balance. They balanced it w/ my flywheel and pressure plate and they marked the elywheel and plate and told me this is how you need to bolt it up but I needed to still get my flywheel turned and that machine shop dosen't turn flywheels so I took it elsewhere and when I got my flywheel back the paint marks were gone for lining it up so I just slapped the pressure plate on the flywheel the first way it would fit. The only thing I haven't gone thru is my rear end. I don't know anything about them. So any help with this issue would be appreciated. Thanks.

The motor mounts are in good shape however one mount looks like a stock mount and the other one is a sold mount. The tranny mount is brand new. Also you say bearings in the rear are we talking the rear wheel bearing or something else. I am new to all this. Also I have pics of the motor mounts if anyone wants to see them give me your email address and I will email them to you.

Check your U joints, maybe 1 is binding a bit or look for grease spray. My front one was causing vibration only at highway speed, around the city no problems.
