stalls after warm up

ok guys.... i have a 1994 5.0. mods: 3:73 gears, bbk cold air intake, under drive pulleys,76mm maf,70mm throttle body, mac equal length shorties, off road h-pipe,flowmaster 1712 american thunder catback exhaust, smog pump delete, msd 6a ignition with msd blaster coil, pro 5.0 short throw shifter, ford racing 1/2" intake spacer. here is my problem: it runs great first thing in the morning, no matter if i go 5 miles or 25 miles to wherever. iget back home, get ready for work and when i fire her back up it stalls at every stop i come to. i've replace about 12 worn out brittle vaccuum hoses and lines, cleaned out my i.a.c., throttle body, sprayed carb cleaner around everything(no loping) checked the torque on my manifold bolts, made sure the egr was operating correctly(dont think its seating all the way)put vaccuum gauge on the manifold and im only pulling around 15 inches. it should be around 20. anyone got any ideas????

I beleive I am having the same problem with my 87 GT. It runs fine but sometimes the idle goes up and down and may cut off. When I come to a complete stop sometimes it cuts off.

I know there are alot of old parts on my car so I will start by replacing my IAC and all other vac hoses and sensors.

When I bought my car it had frogs, lizzards, spiders and wasps in it to let you know how long it sat but it did fire right up and sounded SWEEEEET!

I am sure the pior owner would not recognize it now.
