Confused?!?! any input?

Ok here is my problem...i have a 94 Mustang 5.0 automatic pretty much besides an intake, exhaust, and a shift kit its stock...every time i drive it there is a problem that arises when i hit about 3,000 RPM...(it fluctuates from time to time)...it falls on its face and sticks at 3,000 its almost like my car shifts into neutral by itself i start losing speed and its nearly impossible trying to pull out on a hill its almost lik a 3 year old trying to drive stick.....now i hooked it up to an OBDI/II scanner we have and looked at all the sensers and the computer is saying everything is ok....i put a new cap and rotor, spark plugs and wires on it....and just today a speed control senser...please help, this has me so confused...isit just something stupid im not thinking of or does it seem like i have a serious problem.
