Help with clutch cable adjuster

So i just installed a new clutch and flywheel and the steeda adjustable clutch cable, Now am having a problem with the cable i believe, I adjusted the cable at the fork and tighten the two bolts but for some reason have come lose again so my pedal feels all mushy and vibrates. First time it happen i re-tighten the blots and it stopped the mushy/vibrating clutch pedal. Now today it did it again. Can someone please tell me if i did it wrong, the big bolt goes before the fork hole and the small bolt after or both bolts go threw the fork? Put some lock tight on the threads so it does nt come loose again??? I just dnt want to be dealing with this crap again since its starting to rain.
Thanks guys!!!!!!!!!!!!

Did you tighten the front and back bolts against the fork, so that it was tightly wedged in there?

EDIT: I was a little confused

Looks right to me, It's kind of tough to fit a tool in there but you gotta hold the larger one solid and tighten the other one down at the same time... shouldn't be coming loose at all if you've tightened it good.
Are you running the other adjustable components? i.e. firewall, quadrant and pedal adjuster?

Just hard to believe that it would have come lose again cuz I tighten the crap out of them to make sure it wouldn't come lose. Should I used loc tight to prevent it from coming lose in the future? Would the vibrations make the bolts come loose?

As long as they were tight against each other the vibrations should have made it loose. I would say to use loctite if you have to... the problem is that you shouldn't have to.

The way that me and my friend installed his was, I used a pry bar to get the fork arm out of the way while he used 2 wrenches on both nuts. You loosen the big one and tighten the little one, This makes the 2 push towards each other. Ya see what I'm saying?

So what i feared was true damn it. the nut started to back up making the clutch loose.
This is how it was when i took the cover off. compare with the first pic i posted and you can see the first one has long treads and this one is shorter.

Also i took this video idk is this normal for the fork to vibrate this much? I think it might have been the reason to have made the nut to come loose nt sure. I put in a billet aluminum flywheel.
