Engine surges at start and idle fluctuates

When I started my 95 V6 it made a thumping noise, shot smoke through the air conditioning vents and now will not idle properly. The engine surges to 3000 rpm on start up and then idles roughly even when warm. The transmission also seems to be pulling when I'm braking. I've replaced the idle control valve, but this hasn't had a significant effect on the idle problem. I'd appreciate any advice (besides rolling the car over a cliff, it's got 215,000 miles on it and I want to make it to a quarter million)

check ure vacuum hoses to see if there is a leak any where

as for as smoke coming out of the ac humm

check intake and fuel too. check the fuel filter, air filter, spark plugs/wires, then move on to the throttle sensors and make sure you dont have a leak in the intake manifold.
